Payment Information

 Dr. Guellati-Salcedo's usual and customary fee is $200 per 60-minute appointment for individual sessions. An initial consultation of 90 minutes will be charged $280.

Session packages are available and require full payment upfront, as follows:
- 6 individual session package at $1080 ($180/session).
- 8 individual session package at $1360 ($170/session).
- 12 individual session package at $1920 ($160/session).

For professional services, such as extended phone contact with patient or other professionals (with your permission), preparation of records or treatment summaries, and legal proceedings (i.e., depositions, testimony, preparation and transportation costs), off-hours appointments, fee arrangements will be discussed separately.

Terms of Payment
Please note that payment is due at the time services are rendered, unless other arrangements have been made in advance. 

Current forms of payment include bank-to-bank transfers (via Zelle®​), and Venmo.

About Health Insurance
Dr. Guellati-Salcedo is an out-of-network provider for all insurance plans at this time, except Medicare Part B (this excludes Medicare Advantage plans from private insurers). She will provide a detailed invoice for the purpose of claim submission upon request.  Please, note that coaching sessions are typically not covered by health insurance plans.
